Understanding ALTCS

ALTCS — the Arizona Long Term Care System — is Arizona's Medicaid program for long-term care. For those who qualify, it covers assisted living, memory care, in-home care, and skilled nursing.

Overview

ALTCS is administered by AHCCCS (Arizona's Medicaid agency) and provides comprehensive long-term care to eligible Arizona seniors and adults with disabilities. Eligibility has two components: financial (income and asset limits) and medical (a Preadmission Screening determining need for institutional-level care). Applications are notoriously complex and denials for procedural errors are common.

The medical assessment (PAS) evaluates ADLs, IADLs, cognition, medication management, and continence. Financial rules include a five-year lookback, spousal impoverishment protections, and specialized planning tools such as qualified income trusts (Miller Trusts) when income exceeds the cap.
