Finding the right community is only the beginning. Our Peace of Mind Program provides continued advocacy, education, and clinical care coordination so families can navigate the months and years ahead with confidence.

At AdvaCare, our commitment doesn't end when your loved one moves in. Whether you need help understanding new healthcare services, coordinating providers, advocating during a hospitalization, or simply knowing who to call when circumstances change — your AdvaCare Care Navigator remains available.

The Peace of Mind Program was designed for the reality of modern aging: multiple chronic conditions, cognitive change, complex medication regimens, and families that often live far away. We serve as the clinical translator, the calm voice, and the second set of eyes.

What's included

Concierge care management, delivered with clinical fluency.

Ongoing Care Navigation

Continuous review of your loved one's evolving care needs — from changes in Activities of Daily Living (ADLs) to shifts in cognitive status — and translation of those needs into a clear plan.

Family Advocacy

Questions about care? Need a second opinion? Communication support with community leadership, physicians, and case managers when concerns arise.

Scheduled Community Check-Ins

Recurring follow-up calls and visits to confirm your loved one continues to receive care appropriate to their acuity, preferences, and dignity.

Healthcare Coordination

Warm referrals to trusted clinical partners: home health, hospice, palliative care, mobile physicians, behavioral health, therapy, wound care, private duty, and DME.

Family Communication Support

Coordinated updates for adult children, POAs, and fiduciaries — especially valuable when families are managing decisions from multiple states.

Transitions of Care

Guidance through hospital admissions, rehab stays, level-of-care changes, and — when the time comes — end-of-life care planning with compassion and clarity.

Membership is offered on a monthly or annual basis and can be adjusted as needs change. Final pricing is confirmed during your complimentary consultation.
