Hospital Discharge Planning

How to protect a safe discharge when a hospital or rehab facility gives your family days — sometimes hours — to decide.

Professional Introduction

Understanding Hospital Discharge

Discharge is where most poor placements are made. A case manager hands a family a printed list, a discharge date is set, and decisions that deserve weeks are compressed into a single afternoon.

Families have more rights and more time than they are usually told. You can request a needs assessment, ask for the discharge criteria in writing, appeal a discharge you believe is unsafe, and decline a facility that cannot meet the documented level of care.

This category exists to level the field: what to ask the case manager, how to read a discharge summary, how to evaluate options in twenty-four hours, and how to prevent the readmission that so often follows a rushed decision.

Hospital Discharge: common questions

Can we refuse a hospital discharge we believe is unsafe?

You can request review and appeal a discharge you believe is premature or unsafe. Ask for the discharge criteria and the appeal process in writing, and involve an independent advocate quickly.

Same day in most cases. Discharge situations are the most time-sensitive work we do, and there is never a cost to the family.
