Services

Every kind of senior living decision — handled with care.

From a first tentative phone call to a same-day hospital discharge, our services are designed around one thing: getting the right outcome for the family in front of us. Every service is free.

Assisted Living Placement

For seniors who need support with Activities of Daily Living (ADLs) — bathing, dressing, toileting, transferring, mobility, and eating — in a full-service community with on-site caregivers, medication management, and dining. We match by clinical acuity, personality, geography, and budget.

  • ADL & IADL assessment
  • Medication management
  • Budget & ALTCS awareness

Residential Group Home Placement

Smaller, home-like settings — typically 10 residents or fewer — with a higher caregiver-to-resident ratio. Often a strong fit for higher care acuity, quieter environments, or seniors who thrive with more individualized attention.

  • Personally vetted operators
  • Higher acuity friendly
  • Quieter environments

Memory Care Placement

For seniors living with Alzheimer's disease and other forms of dementia. Secured environments, staff trained in cognitive decline and behavioral expressions, and purposeful programming designed to preserve dignity and quality of life.

  • Dementia-trained staff
  • Secured, dignified settings
  • Family education & support

Independent Living Guidance

For active seniors ready to trade home maintenance for community. Resort-style, quietly luxurious, or 55+ neighborhoods — matched to lifestyle, social preferences, and long-range care planning.

  • Lifestyle-first matching
  • Amenities & culture fit
  • Future-care planning

ALTCS Education & Navigation

The Arizona Long Term Care System (ALTCS) can cover assisted living, memory care, group homes, skilled nursing, and in-home care for those who qualify medically and financially. We translate eligibility, spend-downs, Miller Trusts, and applications — and coordinate elder law counsel when needed.

  • Eligibility walkthroughs
  • Application coordination
  • Elder law referrals

Hospital & Rehab Transitions

When discharge planners give families 24–48 hours, we coordinate rapid, clinically-appropriate transitions of care — vetted options, tours (in-person or virtual), and coordinated move-ins that align with skilled nursing, home health, hospice, or palliative care needs.

  • Rapid response
  • Discharge planner coordination
  • Clinical transition support

Post-Placement Care Management

Our Peace of Mind Program provides ongoing geriatric care navigation after move-in — advocacy, community check-ins, and coordination with home health, hospice, palliative care, mobile physicians, behavioral health, and therapy services.

  • Ongoing advocacy
  • Clinical coordination
  • Family communication support
