The Arizona Long Term Care System (ALTCS) can cover assisted living, memory care, group homes, skilled nursing, and in-home care for those who qualify medically and financially. We translate eligibility, spend-downs, Miller Trusts, and applications — and coordinate elder law counsel when needed.
When discharge planners give families 24–48 hours, we coordinate rapid, clinically-appropriate transitions of care — vetted options, tours (in-person or virtual), and coordinated move-ins that align with skilled nursing, home health, hospice, or palliative care needs.
